Job Description: Home Health Nurse Job Title: Part Time Home Health Nurse Location: South East Houston; Office located in Friendswood Reports To: Director of Nursing Job Summary: We are seeking a compassionate and experienced Home Health Nurse to join our healthcare team at Always Best Care Senior Service, Friendswood. This role involves providing high-quality nursing care to patients in their homes, assisting with medical treatments, and promoting recovery and wellness in a home setting. The ideal candidate will have a minimum of two years of clinical nursing experience, excellent communication skills, and a passion for delivering patient-centered care especially with Seniors. Key Responsibilities: 1. Patient Care: * Assess patients' health status and Create Care plan during initial assessment. * Provide skilled nursing care, including but not limited to wound care, medication administration, IV therapy, and disease management. * Perform physical assessments and monitor vital signs (blood pressure, heart rate, temperature, pulse oxy and much more as needed * Administer injections, vaccines, and other medical treatments as prescribed. * Educate patients and their families about managing their conditions, medications, and health goals. * Coordinate care with other healthcare professionals (doctors, physical therapists, occupational therapy .) involved in the patient's care. 2. Care Plan Development: * Collaborate with physicians and other healthcare providers to develop and implement personalized care plans for patients. * Regularly review and update care plans to ensure progress and adjust treatments as needed. * Provide recommendations to improve patient care and outcomes. 3. Documentation & Reporting: * Maintain accurate and detailed patient records in accordance with healthcare regulations and organizational policies. * Complete required documentation including patient progress notes, medication logs, and treatment plans in a timely and accurate manner. * Communicate patient status updates to the supervisor, healthcare team, and family members as needed. 4. Patient Safety & Compliance: * Ensure adherence to healthcare protocols, infection control measures, and safety standards during patient visits. * Identify potential risks and take appropriate actions to mitigate patient safety issues in the home environment. * Ensure compliance with all regulatory requirements and home health agency policies. 5. Emotional & Psychological Support: * Provide emotional support to patients and their families, offering comfort during challenging health situations. * Offer guidance on coping strategies and mental health care when necessary. 6. Emergency Response: * Respond promptly and effectively in emergencies, including performing CPR or other life-saving interventions if occurring during Home visit. * Assess changes in a patient's condition and escalate care needs as necessary. Required Qualifications: * Education: * Registered Nurse (RN) with a current and valid license to practice in Texas * Minimum of Bachelor of Science in Nursing (BSN) * Experience: * Minimum of 2 years of nursing experience in a clinical or hospital setting. * Home health nursing experience is preferred but not mandatory. * Experience in managing patients with chronic diseases, post-operative recovery, or those requiring long-term care is an asset. * Skills: * Strong clinical assessment skills and attention to detail. * Proficiency in administering medications and treatments according to care plans. * Excellent written and verbal communication skills. * Ability to work independently and manage multiple patient needs. * Strong organizational skills with the ability to prioritize tasks. * Compassionate and patient-centered approach to healthcare * Certifications: * Basic Life Support (BLS) certification. * Advanced Cardiovascular Life Support (ACLS) certification may be required depending on patient needs. * Additional certifications in home health, wound care, or IV therapy are a plus. Preferred Qualifications: * Experience: * 2+ years of clinical nursing experience. * Experience with senior care , home health care is a plus . * * Skills: * Ability to use electronic medical record (EMR) systems for patient documentation; Wellsky is EMR used at Always best care. * Fluency in a Spanish is a plus for patient communication but not mandatory. Working Conditions: * Hours: Part-time, flexible hours based on patient needs. Includes some evening and weekend work. * Travel: Will Travel to patient homes within a designated geographic area. Mileage reimbursement Between client homes. * Work Environment: Provide care in patients' homes, which may include a variety of living conditions. Ability to adapt to different environments and manage non-medical challenges in the home is essential. Physical Requirements: * Must be able to stand, walk, bend, and lift patients or medical equipment as necessary. * Ability to carry out physical assessments, administer medications, and perform other hands-on tasks. * Ability to handle occasional lifting of medical equipment or patient supplies (up to 50 lbs).
