This is a sensitive position responsible for providing Family Planning, Sexually Transmitted Infection care, and Pre-exposure prophylaxis (PrEP) for HIV. The employee shall maintain accurate and complete medical records. The employer, Department of Health, is the records owner of the medical records as defined by section 456.057 Florida Statue: This position working under the direct supervisor of the Community Health Nursing Director - SES with headquarters in Clewiston. The primary duties of this position include: Manages and treats Family Planning, STI, and Pre-Exposure Prophylaxis (PrEP) for HIV clients in accordance with a collaborative practice agreement using established DOH protocols, policies, and procedures. * Assesses, evaluates, and manages patients in accordance with current APRN professional practice standards, evidence-based standards, and medical treatment guidelines and in consultation with supervising physician when indicated * Documents all observations and actions accurately and in accordance with program guidelines, including prescribed timelines * Collects data pursuant to and evaluates patient complaints, medical histories, and physical findings * Orders diagnostic laboratory tests for evaluation and management of patients. Interprets and evaluates laboratory results * Provides preventive health care services in relation to programmatic guidelines * Evaluates and initiates or modifies the patient treatment plan * Prescribes medication and accurately documents medication in the electronic medical record * Provides referrals for physician or specialty care when indicated * Performs testing procedures, venipunctures, U/A, pregnancy test, HIV testing and counseling. Initiates and manages contraceptive methods for Family Planning patients after evaluation, history, laboratory tests, physical assessment, and examination. * Provides verbal and written instructions regarding family planning methods * Obtains informed consent * Performs assessment, screening, and counseling. * Obtains Pap smears and provides follow-up for abnormal Pap smears * Provides pre-conceptual and inter-conceptual health services * Inserts and removes IUD's and implantable subdermal contraceptives * Assists patients to make decisions regarding use of oral contraceptives, Depo-Provera injections, natural family planning, and long-acting reversible contraceptives * Provides pregnancy testing and counseling * Manages and treats sexually transmitted infections (STI) * Collaborates with the Disease Intervention Specialist to ensure STI treatment is received and documented * Provides sterilization counseling * Follows Family Planning Title X Administrative and Clinical Manuals and protocols guidelines * Collaborates with the Refugee Health nurse to provide physical examinations and follow-up referrals as warranted * Documents all observations and actions in an accurate and timely manner in the Health Management System (HMS) * Responds to patient complaints Provides information/education to clients and/or their families upon a diagnosis, including a plan of care, lifestyle, compliance, health practices, medications, complications, and follow/up. Participates in orientation and staff development of personnel and assists with patient care. Functions as a resource for care information for nursing staff and other health care professionals. Coordinates Tubal Ligation program. Attends DOH staff and programmatic meetings. Utilizes a total quality management approach to provide efficient and effective services that meet organizational, customer service, internal process, and financial goals * Assesses the health and health care needs of adults and children and makes service delivery recommendations based on quantitative and qualitative data as well as feedback from community partners and consumers * Actively participates in performance and process improvement initiatives including quarterly chart reviews. Serves as a backup in providing public health nursing services, immunizations, and related health unit activities at scheduled clinics.
