Case Manager - RN Transitional | Case Management | Days | Part Time


Overview The RN Complex Case Manager will work in a Transitions of Care model. This position assesses, plans, implements and monitors a comprehensive plan of care for her/his group of patients. Identification of discharge barriers and complex patients needing focused care management by collaborating with other members of the medical team, i.e. physicians, social workers, Case Management, PT/OT staff, pharmacists, respiratory, dietary, and nursing. The complex Case Manager will provide Care Transitions for Florida Blue Patients that have high risk for readmission and work with post-acute care teams to prevent 30-day readmissions. They will provide education for disease management and medication and include any special recommendations for discharge to SW and CM. The position will require working with outpatient Care Transitions teams to provide handoff and care plan for FLBL patients. Qualifications Minimum Education and Experience Requirements::Licensed RN in Florida. BSN preferred. 2 years recent RN experience (in a clinical capacity) applicable to the designated population served is required. Prior Case Management experience is preferred. Case Managers hired after January 1, 2018 who do not possess a BSN and/or an MSN will be required to obtain, at a minimum, a BSN within three years of employment as a Case Manager. Professional certification in specialty area preferred. Case Managers hired after January 1, 2018 will be required to obtain specialty certification within two years of employment as a Case Manager.Motor Vehicle Operator Designation::Employees in this position:Will not operate vehicles for an assigned business purposeLicensure/Certification/Registration::Valid RN license in the State of Florida.

$40.53/hour
