The centers provide campus-based direct services and supports at 13 centers located throughout the state. The Physical Nutritional Management Plan (PNMP) Coordinator works under the general supervision of Unit administration and is stationed in residential and program areas. Habilitation Therapies provides training and supervision for technical duties, clinical applications, and documentation and training issues. Work hours are normally day or evening shifts with variable responsibility for night shift activities. The PNMP Coordinator provides service to individuals in the areas of physical and nutritional management, oral and enteral eating, positioning, mobility, communication, and other related PNMP services. Duties include conducting competency-based training of staff; monitoring programs; monitoring availability, condition, and proper use of assistive equipment; and ensuring appropriate availability and condition of PNMP instructions and illustrations. The PNMP Coordinator works under moderate supervision with limited latitude for the use of initiative and independent judgment. Other duties may be required which are reasonably related to the essential or marginal functions of the position. Specific duties include but are not limited to: attending PNMP meetings and clinics, NMT (Nutritional Management Team) meetings, and PST/HST (Personal Support Team/Health Status Team) meetings as requested; filing PNMP documentation such as PNMPs, feeding illustrations, positioning photographs, and data sheets; training Direct Support Staff on changes in PNMPs, positioning, lifting, transferring, and meal time practices; monitoring lifting, positioning, transfers, walking programs, use of communication devices, and mealtime supports; and reporting problems to the appropriate home supervisors, Unit administration, and Habilitation Therapies. The PNMP Coordinator will provide new employee orientation and refresher training as indicated and will provide direct training of new employees with specific individuals served in lifting, mealtime procedures and other PNMP related activities/plans. This position may be OR is required to drive a state-owned vehicle. Essential Job Functions: Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned. Provides competency-based training and mentoring to ensure staff is well prepared and knowledgeable about activities outlined in the PNMPs in order to ensure Plans are carried out as prescribed. Serves as a member of the PNMP Team to ensure PNMP information is accurate. Maintains pertinent documentation in PNMP notebooks in a timely manner to keep Direct Support Staff informed of PNMP changes and trains staff in changes. Monitors all aspects of PNMP related programming, quality of implementation, and staff compliance and reports findings to QDDP, Unit RN, and HT in order to ensure continued effectiveness of programs. Gives input into development of PNMPs for assigned individuals to meet needs agreed upon by the IDT. Monitors meals and snacks to ensure safe eating practices. Provides assistance, as needed, in transporting of individuals going on scheduled activities and in emergency situations. Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location. Knowledge Skills Abilities: * Must be able to model activities outlined in the PNMP in the prescribed fashion. * Must have acceptable writing skills. * Must be able to express ideas verbally in clear and orderly fashion. * Must be able to establish and maintain rapport with individuals and co-workers. * Must have ability to maintain flexibility in work schedule, be self-motivated with strong organizational skills. Registration or Licensure Requirements: Valid Texas driver's license or ability to obtain it no later than 90 days after hire date, is preferred. Applicants with an out-of-state driver's license must provide an original certified driving record from the state of driving licensure. Eligible driving record required based on HHSC Fleet Initial Selection Criteria: Experience in direct care work. Experience in working with people who have developmental and intellectual disabilities. Graduation from a standard senior high school or equivalent is generally preferred. Experience and education may be substituted for one another. Experience in writing reports preferred. Additional Information: Applicants must pass a fingerprint criminal background check, pre-employment drug screen, and registry checks including the Client Abuse/Neglect Reporting System (CANRS), Nurse Aide, Medication Aide and Employee Misconduct and HHS OIG List of Excluded Individuals/Entities. Flexibility in work hours may be required for this position. The position may be required to work overtime and/or extended hours. Must be willing to work weekends and holidays.
