Someone with your depth of compassion, clinical insight and strong decision-making capabilities is the ideal person to map out and oversee the process of a patient's recovery from illness, injury or surgery at home. * Reporting to the Coordinator of Care Nurse, you will be responsible for seeing patients in their home for revisits based on a skilled need. * You will implement the plan of care and evaluate the treatment. * You will document the visit electronically and completing documentation within 24-48 hours. Qualifications: * NY State LPN License required. * Minimum of one year Med/surg experience in a hospital or Longterm Care facility required. * Must have a NY State Drivers License and a car. Apply
