Job Description Position Summary: The Nurse Navigator is responsible for performing nursing assessments and triage of patient care needs for new and ongoing patients, including patients on a clinical research study. Oversees the planning, scheduling, and implementation of day-to-day clinical activities and procedures for navigated patients. The position will work side-by-side support a provider daily. Essential Job functions: * Timely navigation of patients across the continuum of care beginning with diagnostic to include coordinating care with staff and providers in other clinical departments. This could include o Genetic Counseling and Cognitive Therapy, and refers to community resources to facilitate timely diagnosis, treatment, and survivorship. o In patient and outpatient coordination of care management of chronic, preventive health, and transitional care needs. * Assesses, plans, and provides nursing care, counseling and education to patients providing physical, psychological, and emotional support to patients and their family members. * Responsible for providing telephone triage, advice, and health education. * Answer patient calls on a wide range of topics, asks the right questions to get the information needed to make an accurate assessment. * Ability to assess the problem, based upon the conversation, and use clinical judgment, knowledge and experience to direct the conversation and guide in the decision to give self-care advice, refer the patient to the providers, make an appointment or call 911 for emergency assistance. * Delivers direct patient care within the scope of licensure and in accordance with Summit policy. * Communicates with physicians and other members of the interdisciplinary team, within Summit, as well as with outside providers. * Facilitates scheduling of appointments with staff to ensure timely referral and transition into specialty services. * Documents all patient visits and communication in electronic medical record. Manages and submits statistical data monthly. * Develops an expertise in the department protocols to support patients during their care. * In depth knowledge and understanding of disease processes, drug reactions and preventive care. * Other duties as assigned. Cardiology General and/or Pulmonology Department ONLY: * Responsible for assessing patients with heart failure * Coordinating rapid access to care as needed * Hospital admission and discharge oversight * Patient education, including symptom management Oncology/Hematology Department ONLY: * Support and cross train into chemotherapy * Responsible for assessing patients with cancer diagnosis * Coordinating rapid access to care as needed * Hospital admission and discharge oversight * Patient education, including side effects of chemotherapy * Places orders for medications to alleviate symptoms of chemotherapy admissions * Navigates and schedules patients for surgery, chemotherapy, and radiation therapy General Job functions: * Participates in community outreach programs. * Coordinates the availability of patient education materials as well as being familiar with resources available in the community. * Assists Clinical Manager with the development and implementation of Department Programs in multiple areas (e.g., survivorship, cardiology heart failure, patient education materials, identification of community resources, marketing, prioritizing workflow). Environmental Risks: * Allergens: dust, mold and/or pollen * Combative Patients / Visitors Education, Certification, Computer and Training Requirements: * State of Oregon RN license * BLS Certification required * Ability to communicate in English, both orally and in writing * Experience with Epic preferred In addition to the above requirements, if position is in the Cardiology Specific, the below are also considered: o 3 years cardiology and Cardiac Congestive Heart Failure experience preferred o Case management and/or remote patient management experience preferred o Heart Failure (CHFN) Certification through the America Association of Heart Failure Nurses preferred o ACLS Certification preferred In addition to the above requirements, if position is in the Pulmonology Specific, the below are also considered: o 2+ years of experience with complex pulmonary disease o Education opportunities available relating to Pulmonary Hypertension and Interstitial Lung Disease o ACLS Certification preferred In addition to the above requirements, if position is in the Oncology/Hematology Specific, the below are also considered: o 3 years' oncology experience preferred o OCN preferred o ONS Chemotherapy Biotherapy Certification required within 90 days of hire/transfer About Our Commitment Total Rewards at VillageMD Our team members are essential to our mission to reshape healthcare through the power of connection.
