ED Care Management Specialist


POSITION SUMMARY: Cross-trained position including Utilization Management/Care Management roles. Responsible for reviewing documentation in the chart for appropriate status and coordinating the care of patients meeting case management criteria across a continuum of care.# Ensures and facilitates the achievement of quality, clinical and cost outcomes.# Assesses, plans, and coordinates the discharge planning process for patients, as well as arranging post-acute care services for patients in a timely healthcare delivery.# Additional responsibilities include serving as a patient advocate, utilizing resources efficiently to improve patient outcomes. # EDUCATION, CREDENTIALS, TRAINING and EXPERIENCE: Minimum Required: ED Care Management Specialist:# Graduate of an accredited school of professional nursing, with current license to practice as a Registered Nurse in North Carolina. Thorough knowledge of nursing care process from admission through discharge, gained through several years of clinical experience as a RN. # Preferred:#ED Care Management Specialist:# Baccalaureate Degree in Nursing and previous experience as a Care Manager or Utilization Review Specialist.# Knowledge of utilization review and discharge planning methods and procedures.# Understanding of clinical pathways and disease management models. # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # # ADDITIONAL SPECIALIZED KNOWLEDGE/SKILLS:# Incumbent must demonstrate an ability to apply age-appropriate principles of communication and clinical practice relevant to neonatal, infant, pediatric, adolescent, adult and geriatric patients.# Incumbent must demonstrate competence in the use of equipment and application of procedures before being allowed to perform independently.
