Job Summary CareFor is currently looking for a Bachelor's prepared RN to join our Care Management team in a full-time capacity. Nurses who do well in our environment have experience with developing long-term relationships with patients (and their support network ' children, siblings, spouses, close friends) and enjoy caring for patients across the continuum of care, in many different settings. We are looking for a self-motivated and organized nurse who is comfortable setting their own schedule and willing to work as a member of a dynamic team. All of our current nurses enjoy both the clinical and relational aspects of their job. We are looking for someone who would appreciate that as well. The Nurse Care Manager is a Registered Nurse (RN) who develops, implements, and evaluates individualized care plans, acts as a patient advocate, and serve as a liaison between patients, their families and other healthcare providers to meet the needs of the individual in the home and community setting. Why work for CareFor? * Competitive Pay * Health Benefits * Generous Paid Time Off program * Paid orientation and training * Recognition & Awards * Opportunities for growth and career progression Responsibilities * Complete an initial assessment of the patient to determine needs including medical, social, family, living arrangement, or any pertinent daily management, and reassess as needed. * Complete a health assessment to determine nursing needs. * Develop, plan, implement, and evaluate an ongoing care plan to meet individual needs. * Make home visits according to the patient's individual needs, and modify visit frequent as needed. * Notify the physician of change in condition. * Coordinate care with other healthcare or community providers such as home health, hospice, nursing facilities, physicians, therapists, or hospitals to ensure patient's needs are met. * Accompany patients to physician appointments as needed to ensure optimal healthcare, and advocate for patient needs. * On-Call Rotations Qualifications * Will be a registered nurse (RN) licensed in Texas or in accordance with the Texas Board of Nursing rules for Nurse Licensure Compact (NLC). * Will have at least one-year experience as an RN in a clinical role. * Will be able to demonstrate effective written and verbal communication. * Will be able to read, write, and comprehend English. * Will be able to comfortably navigate an Electronic Medical Record, knowing basic computer skills. * Will be able to triage patient needs and prioritize a daily schedule, knowing that needs change throughout the day * Will be able to critically think to solve problems creatively * Will be able to lift, bend, squat the level and amount determined by each job assigned. * Will present proof of a current driver's license and have a dependable insurance automobile. * Will be neat in appearance. * Will be at least (18) years of age.
