Rosado at [email protected] SUMMARY OF POSITION: Functions as part of an interdisciplinary team to maintain continuity of patient care for identified patient populations from admission to discharge utilizes appropriate criteria for inpatient care and plans timely discharges. Responsible for coordinating patient's clinical plans of care guided by Patients/Families, Physician Providers, and other members of the inpatient team while adhering to guidelines and contracts from regulatory bodies and insurers. MINIMUM QUALIFICATION(S): * Two (2) years nursing experience * Currently licensed to practice as a Registered Nurse by state of employment or holds a multistate RN license through the interstate Nurse Licensure Compact. * AHA BLS before end of the orientation period PREFERRED QUALIFICATION(S): * BSN preferred. * Previous experience in case management, care coordination, outcomes management or discharge planning preferred. * Certification in case management from a nationally recognized organization preferred.
