HIV Health Services Center (HHSC) is a part of Primary Care. HHSC serves persons with HIV in the six-county Portland Metropolitan area. HHSC provides ambulatory outpatient medical care (including mental health and substance abuse counseling) case management, health education, HIV prevention and risk reduction and treatment adherence counseling. Our comprehensive, multidisciplinary model is tailored to meet our patients' complex health needs. Access to and use of HIV medications is optimized by clinical pharmacy services on site. Because of the infectious nature of HIV and public health mandates, HHSC integrates HIV prevention messages into all aspects of service delivery. HHSC is an AIDS Education and Training site and is involved in various research projects. The Community Health Nurse provides comprehensive community and clinic health services directed towards the prevention of disease, the promotion of health, and the clients self care using professional judgment in comprehensive assessment, screening, nursing diagnosis, care planning, counseling, intervention, case management, health information /education, referral and evaluation. This position requires extensive knowledge of HIV/AIDS in a wide variety of complex patient scenarios that involves working with homelessness, substance use and mental illness disorders. The ability to multi-task while providing intensive case management with advanced nursing assessment skills are essential. HIV Nursing practice incorporates harm reduction strategies to promote health promotion. As the Community Health Nurse, other duties will include: HIV Care Management * Comprehensive Health Management: Proactively assesses, plans, and manages various health conditions (HIV/AIDS, Hepatitis, Substance Use Disorder, etc.) to improve patient outcomes through screening, intervention, and care coordination. * Patient-Centered Collaboration: Partners with the healthcare team and community resources (Medical Case Managers, Navigators, etc.) to develop personalized care plans and actively involve patients in their own health decisions. * Strong Care Coordination & Outreach: Ensures seamless care transitions by coordinating referrals, following up with patients post-discharge, and collaborating with external healthcare providers and agencies. Team Collaboration * Comprehensive Clinical Care: Assesses and addresses urgent/non-urgent medical needs, provides health education, administers treatments, and prescribes medications based on standing orders and protocols. * Collaborative Team Leadership: Acts as a resource for the healthcare team, collaborating with various professionals (case managers, navigators, pharmacists, etc.) to ensure coordinated and effective patient care. * Specialized Expertise & Ongoing Development: Maintains clinical skills relevant to HIV/AIDS adult primary care, OB, and internal medicine, participates in daily case reviews, and completes required certifications (ACRN, Point of Care). All non-traditional education, training and experience will be evaluated. * Possession of, or ability to obtain, an appropriate and valid license to practice as a Registered Nurse issued by the State of Oregon. * One of the following: * A Bachelor's degree in nursing from an accredited nursing program; OR * An Associate's degree in nursing from an accredited nursing program AND two years of registered nurse level experience is required. * BLS/CPR certification required * Ability to work independently * Ability to work in an interdisciplinary setting fostering a safe, welcoming and inclusive environment * Experience providing services to diverse and vulnerable communities * ACRN certification, or ability to obtain ACRN certification within 6 months of hiring.
