Nurse RN
UPMC Home Health is Hiring a Nurse RN for Westmoreland County and Eastern Alleghany area. The Professional Staff Nurse is responsible for the delivery of skilled nursing services to patients in their homes according to the physician's plan of care, agency policies and procedures, fiscal intermediaries, regulatory agencies, and bodies, and within recognized standards of nursing practice. Must possess the ability to assess and interpret patient data needed to identify each patient's requirements relative to the population served. Establishes and maintains collaborative relationships with physicians, other health care providers, patients, and their families, to achieve desired patient outcomes throughout the continuum of care. Assignments will be determined by the Clinical Manager of the branch based on several different factors, which include, but are not limited to, coverage for Leave of Absences, increase in patient volume in a specific team, and vacancies. Responsibilities: * Coordinates case management activities within an interdisciplinary health care team to ensure appropriate delivery of services to patient.Attends compliance training and adheres to the organization standards of conduct, policies, and procedures.Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.Completes an initial and comprehensive assessment of patient.Constructively receives feedback and direction. Takes action to improve knowledge, skills and performance based on feedback or on self-identified developmental needs. * Initiates, coordinates and ensures ongoing communication with the physician and other members of the interdisciplinary health care team, including evaluating patients and family centered health teaching.Provides direct patient care, evaluates outcomes, and adjusts nursing care process as indicated to ensure optimal patient care. Establishes the plan of care in collaboration with the physician, patient, and interdisciplinary team members in accordance with the physician orders and patient needs.Assigns other interdisciplinary team members as appropriate to assist in the appropriate delivery of services as ordered on the plan of care to the patient.Supervises and teaches other nursing personnel including LPNs and Home Health Aides as needed. * Attends and participates in case conferences, team meetings, and other work-related meetings.Recognizes limitations and requests assistance and direction to meet performance goals.Completes all necessary documentation which may include visit assessment, plan of treatment, and verbal orders accurately and promptly in electronic documentation system while in the patient's home and within the agency's and regulatory standards. Data syncs (transmits) information same day.Identifies, investigates, and recommends solutions to organization issues. * Exercises independent judgement in matters concerning emergent and non-emergent patient care needs and communicates with the physician.Identifies, develops and participates in process improvement opportunities within the agency that will enhance the quality of the services we provide.Participates in weekend and on-call coverage rotation for the agency as per BU guidelines.Demonstrates knowledge and understanding of the UPMC standard of care delivery and proficiency in all aspects of patient care, to include complex and/or specialized care. Qualifications: * The individual must be able to demonstrate the knowledge and skills necessary to provide care and/or interact appropriately to the ages of the patients served by his/her assigned unit as specified below. They must also demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patients' status and interpret the appropriate information needed to identify each patients' requirements relative to his/her age-specific needs and to provide the care needs as described in the department policy and procedures. * Zero (0) to two (2) years of experience preferred. Six (6) months of nursing experience in an Acute Care or Skilled Care Facility or Home Health or Hospice Care preferred. * BSN preferred. Ability to establish and maintain positive, caring relationships with executives, managers, physicians, non-physician providers, ancillary and support staff, other departments, and patients/families. Ability to work productively and effectively within a complex environment, handle multiple/changing priorities and specialized equipment. Good clinical judgment with critical thinking, analytical and problem-solving abilities required as related to various aspects of patient care. Critical thinking skills necessary to exercise and to lead others in application of the nursing process. Mobility and visual manual dexterity. Physical stamina for frequent walking, standing, lifting, and positioning of patients. Successful completion of Home Health/Hospice Orientation upon hire. Licensure, Certifications, and Clearances: Current licensure as a Registered Professional Nurse either in the state where the facility is located or in a state covered by a licensure compact agreement with the state where the facility is located. CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire. Graduate nurses must complete licensure examination within SIX (6) MONTHS of hire or within one year of graduation, whichever comes first. Though temporary permits are valid for one year from the date of graduation, not one year from issue of permit (or until the results of the examination are known at which time it becomes null and void), UPMC requires that GNs take the licensure examination within 6 months of their start date or they will be terminated or demoted from the GN position, at the discretion of the BU. * Automotive Insurance * Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR) * Driver's License * Registered Nurse (RN) OR Temporary Practice Permit (TPP) * Act 33 * Act 34 * OAPSA * Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
