Job Code Senior Registered Nurse 1-3 (21254Y-21256Y) Recruitment Pool All Applicants Posting Number req6549 Unit School Of Medicine MBU Department School of Medicine, Internal Medicine, Hematology/Oncology Department Website Link Location VCU Address MCV Campus, Richmond, VA 23284 USA Duties & Responsibilities Bleeding Disorders Nurse Case Manager School of Medicine, Internal Medicine, Division of Hematology/Oncology Position Summary: To provide case management including the development and implementation of nursing care plans and the coordination of multidisciplinary services for patients with bleeding disorders and their families at the VCU Hemophilia Treatment Center (HTC). This includes leadership in pre-admission and discharge planning to coordinate bleeding disorders related care as well as independent decision making and problem solving in the triaging of patient calls according to standards of care. The position provides outreach and education to patients and families to improve home care outcomes and provides education to other health care providers. The position also recruits and manages patients on clinical research and clinical trials. Core Responsibilities: Responsibility 1 (55%): Case Management. * Task 1: Conducts nursing assessments, develops and implements plans of multidisciplinary care for pediatric and adult patients and their families in and outside of comprehensive bleeding disorders clinic. * Task 2: Collaborates with home infusion companies and insurance carriers to ensure that patients have access to home medications in a timely and coordinated manner. * Task 3: Plans for pre-admission and discharge bleeding disorders related care. * Task 4: Triages and problem solves complex patient and family issues, communicating with other team members where appropriate. Identifies options and develops solutions for unique or unprecedented situations and follows up with families to ensure maximal outcomes. * Task 5: Effectively communicates with other health care providers to coordinate and implement plans of care * Task 6: Provides patient and family education about their bleeding disorder and home care management, including the development and implementation of teaching and learning materials. Triages patient calls to assist families with problem-solving related to home care. * Task 7: Assesses the readiness of patients for home therapy and teaches self-infusion or injection where applicable. Provides new patient and family education, developing and modifying teaching aides to maximize learning. Responsibility 2 (20%) Public Health Outreach and Education * Task 1: Collaborates with local hemophilia chapter to provide education to families regarding bleeding disorder state of the art care. * Task 2: Provides outreach to community health care providers to enhance awareness of bleeding disorders and to reach underserved or unserved populations. * Task 3: Reaches out to patients and families to improve adherence to bleeding disorders care and identifies and problem solves methods to ameliorate barriers to care. * Task 4: Communicates effectively with schools, daycare providers, long-term care facilities to ensure coordination of care and provides education and resources. * Task 5: Explores alternatives to the delivery of care, including satellite clinics, and participates in state and federal health initiatives to improve patient access and outcomes. * Task 6: Provide clinical leadership for grant services and collaborate with VBDP team and VDH leadership on the public health mission of the program. Responsibility 3 (15%): Professional Education * Task 1: Develops and provides in-house training programs, in-services, educational tools, and state-of-the-art information/resources to healthcare providers (HCPs) within VCU Health and external medical centers. Enhances awareness of inherited bleeding disorders, and ensures best practices are followed by providing mentoring, coaching, and training as needed. * Task 2: Engages in professional development by attending relevant conferences, seminars, and training programs to stay current with advancements and best practices in bleeding disorders care. Additionally, this role provides opportunities to present at state and regional conferences to promote awareness, share knowledge, and contribute to the continuous improvement of patient care. * Task 3: Triages referrals to HTC from community HCPs and provides follow up to promote best outcomes. Responsibility 4 (10%): Coordination of clinical research and public health initiatives. * Task 1: Emphasizes alignment with institutional and ethical guidelines for patient enrollment. * Task 2: Focuses on educating patients and ensuring ongoing engagement in research, beyond just informed consent. * Stresses compliance with regulatory requirements as well as good clinical practices. * Highlights collaboration with both internal and external stakeholders for public health data initiatives. Qualifications Minimum Hiring Standards: * Standard 1: Registered nursing license in Virginia * Standard 2: Bachelor of Science in Nursing Degree * Standard 3: Experience in patient care management, preferably in a clinical or public health setting * Standard 4: Demonstrated experience working in and fostering a diverse faculty, staff, and student environment or commitment to do so as a staff member at VCU * Standard 5: Strong interpersonal and communication skills with the ability to collaborate across multidisciplinary teams Preferred Qualifications: * Standard 1: Five years of relevant professional experience or combination of education, training and experience, preferably in hematology * Standard 2: 1-2 years of experience in clinical research or public health surveillance * Standard 3: Previous experience in case management and education * Standard 4: Knowledge and experience in public health, health policy, and public and private health insurance markets, with the ability to review and interpret policies, and skills in policy analysis and healthcare system navigation * Standard 5: Experience in policy review and interpretation, especially in relation to state and federal health initiatives Additional Information: Open Until Filled.
