Location: Raker Center - LTC Skilled Nursing Facilities (99 beds) Monday-Friday (No weekends, holidays or on-call) A 99-bed facility, Raker Center is open to residents ages 18 and older with primary diagnoses including, but not limited to, cerebral palsy, muscular dystrophy, quadriplegia or multiple sclerosis. An interdisciplinary team seeks to help residents maintain as much independence as possible for the longest amount of time. The team consists of attending physicians, neurologists, psychiatrists and psychologists, RNs and LPNs, social workers, chaplains, and physical, speech, occupational and recreational therapists. * JOB SUMMARY * Completes resident assessments as required by state and federal regulations. Assists in ensuring that clinical staff have the knowledge and skills necessary to provide high quality, competent, and safe care to residents. Primarily Medicaid (MA) residents with a case mix index (CMI) of 1.23. * ESSENTIAL FUNCTIONS * UTILIZES STATE & REGULATORY GUIDELINES FOR RESIDENT ASSESSMENT TO SUPPORT CARE PLANNING AND FINANCIAL REIMBURSEMENT * Is self-directed and responsible for completing all required documentation * Maintains rapport with interdisciplinary team members, residents, and families and demonstrates the ability to communicate effectively with the team. * Completes MDS assessments in a timely manner in accordance with the MDS calendar. * Assures supporting documentation and assessment data is an accurate representation of each resident. * Signs MDS for completeness. * Monitors CNA documentation for completeness and accuracy * Provides ongoing education to CNAs to promote better understanding, increased accuracy, and reliability of their documentation. * Maintains knowledge of RAI manual changes for accurate completion of OBRA and Medicare assessments. * Works in collaboration with FRANC and assumes her responsibilities when not present, by: * Completing the scheduling process for all OBRA and Medicare assessment types * Completing the process for submission of MDS data to the federal database * Demonstrating awareness of resident eligibility for Medicare SNF coverage * Demonstrating awareness of CMS proposals for the budgeting process * Audits completed MDS for accuracy in absence of FRNAC * Assists Nurse Managers with care planning and facilitates care plan meetings when the Nurse Manager is unable to attend. * ASSURES LEARNING NEEDS OF CLINICAL ASSOCIATES * Assists in the development of in-service programs in response to clinical needs and teaches staff according to current accepted standards of care/practice as well as Good Shepherd policy. * Conducts orientation, preceptor and mentoring programs for employees * Assists with monitoring compliance with attendance and competency achievement for associates, which includes feedback to both staff and department heads * Collaborates with clinical management teams to identify specific clinical education needs * Participates in developing Best Practice Standards to assure patient safety and quality of care * Works collaboratively to develop and implement QAPI programs * Completes rounding on LTC nursing units in an effort to review clinical skills and monitor care delivery * Acts as a preceptor for new and current associates and students * PARTICIPATION IN NURSING OPERATIONS * Maintains all aspects of HIPAA requirements * Participates as an active member of the LTC Nursing Management Team * Assists nurses in identifying changing resident conditions and implanting appropriate follow-through * Participates in setting Best Practice standards to assure quality of care * Supports clinical practices that adhere to the organization's philosophy, goal, standards of care, and required governing regulations * ASSURES QUALITY OF CARE * By assisting in the development and interpretation of LTC and nursing philosophies and standards of care * Enforces adherence to the state board of nursing, nurse state practices, and all requirements and other governing agency regulations, such as DOH, Joint Commission, and DPW * Demonstrates skill in understanding the range of treatment and knowledge of the principles of growth-development for the age of the residents served * Obtains quality measure reports and distributes the same to all appropriate staff as required. * MAINTAINS PROFESSIONAL AND TECHNICAL KNOWLEDGE * Attends educational workshops, receiving and reviewing professional publications, establishing professional networks and participating in professional societies. * Maintains RAC-CT and AANAC memberships * ESTABLISHES A COMPASSIONATE ENVIRONMENT * Provides emotional, psychological and spiritual support to residents, families, caregivers, friends and other associates. * PROVIDES INFORMATION TO RESIDENTS AND HEALTH CARE TEAM * Informs an interdisciplinary team of changes in regulations and in the RAI manual * Attends nurse management meetings, staff meetings, and resident Town Hall meetings * Attends interdisciplinary team rounds with Nurse Managers as needed. * RESOLVES RESIDENT NEEDS * Uses interdisciplinary team strategies, communicating to families, residents, caregivers as needed. * ASSISTS FRANC WITH MONITORING OF DOCUMENTATION OF RESIDENT CARE SERVICES * Monitors complete forms with licensed and unlicensed staff and interdisciplinary team members to assure complete and competent results as directed. * Assists with development of forms to adequately meet LTC regulations and the facility's needs. * DEMONSTRATES LEADERSHIP SKILLS * Acts as a role modeling and meeting facility objectives in personnel management * Effectively adapts and adjusts to new or changing conditions, work pressures or difficult situations. * Assumes responsibility for self-evaluation and educational needs * PROTECTS RESIDENTS AND STAFF * Monitor infection control practices to determine whether the appropriate measures are identified, implemented and documented. * CONTRIBUTES TO ORGANIZATIONAL TEAM EFFORT * By accomplishing related results as needed. *
