  • Mondays: 7:30am - 5:00pm (Old Town Clinic; Clinic Triage) *
  • Fridays: 7:30am - 5:00pm (Old Town Clinic OR Blackburn Primary Care Center; Phone Triage) MINIMUM QUALIFICATIONS (REQUIREMENTS) * Note: Prior to submitting your application, please ensure your professional/working experience reflects the requirements listed below: * Required: Successful completion of a course of accredited study that leads to the AD, or BS or AA degree in Nursing from OSBN or State Board approved course. * Required: Current RN license in the State of Oregon, as issued by the Board of Nursing. SUMMARY: The Clinic Triage RN and Desktop Triage RN role works collaboratively as a member of a multi-disciplinary team providing medical and behavioral health services and care coordination for complex, medically vulnerable adults experiencing houselessness. Provides direct nursing care, with an emphasis on triage, education and coordination for patients as outlined by the Clinical Services Manager (RN). Additional responsibilities include wound care and combined provider/nurse visits for substance use treatment. ). Specific responsibilities may vary based on current clinic and team needs. *NOTE: This role may be split between onsite and remote work depending on staffing needs. CLINIC/TRIAGE - ESSENTIAL DUTIES AND RESPONSIBILITIES: * Use nursing assessment and clinical skills in an integrated primary care setting that includes substance use and mental health treatment. * Participate as a member of the interdisciplinary team to provide integrated patient care. * Act as a champion of clinical best practices from a nursing perspective. * Assess and triage patients in-person and by phone. * Provide nurse visits, provider/nurse co-visits with utilization of nursing protocols. Visits include nursing assessment, documentation in the electronic health record and patient education. * Cover clinic wound care schedule as needed and provide wound care according to established treatment plans. * Provide patient education. * Administer medications per provider order, with appropriate documentation. * Respond to urgent in-person and telephonic situations. * Administer treatments including but not limited to wound care, suture removal, catheterizations. * Provide clinical and operational oversight to medical assistants and other clinical and support staff. * Lead problem solving within the interdisciplinary team. * Collaborate with team members across Blackburn Center, Central City Concern, and outside of the organization. * Support high-quality patient experience and address concerns of patients and external partners related to visit experience. * Provide medication monitoring, in partnership with pharmacy, in accordance with participant's care plan. * Provide appropriate health care screenings as indicated. * Support completion of referrals and coordinate care. * Support community partners in furthering understanding of the medical and psychosocial sequelae of homelessness and the systems and processes used to help complex and medically vulnerable participants secure and maintain stable housing, healthcare, and other social services. * Participate in compliance, quality assurance and quality improvement activities as directed. * Work as a member of the Blackburn Center multi-disciplinary team to deliver comprehensive, client centered, and trauma informed integrated services. * Actively participate in the integration of services at the Blackburn Center and identify new opportunities to improve collaboration between and within service and care teams while carefully following information-sharing guidelines. * Proactively identify opportunities for improvement within Blackburn Center workflows and logistics. Participate in review of Blackburn Key Performance Indicators; review and contribute to continuous improvement opportunities and plans. * Build understanding of and appreciation for diversity among Blackburn Center clients and support their connection to culturally responsive and culturally specific services as requested. * Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee. * Other patient care duties as assigned and within scope. DESKTOP - ESSENTIAL DUTIES AND RESPONSIBILITIES: * Daily phone triage utilizing Clear Triage software and receiving warm handoffs from clinic staff. * Will use triage protocols and clinical decision making to determine the level of care required for patient. SKILLS AND ABILITIES: * Ability to consider the impacts and outcomes for underserved communities during decision-making processes. * Ability to consider impacts of systems of oppression, structural racism, and individual bias on client outcomes. * Demonstrated ability to adhere to trauma-informed care principles. * Experience with care for individuals with complex substance use and mental health backgrounds. * Ability to exhibit strong nursing judgment in the care of complex participants and knowledge of nursing theory and practice. * Working knowledge of principles, practices and procedures of outpatient care and programs and services available to participants. * Ability to be flexible and to coordinate and relate effectively with members of several interdisciplinary teams, participants, supervisors and co-workers. * Ability to provide leadership. * Ability to work independently, prioritize the workload, problem-solve, and complete tasks in a timely fashion. * Ability to multi-task and remain calm in a busy and stressful environment. * Excellent oral and written communication skills. * Ability to maintain strict standards of confidentiality with regard to patient information. * Ability to maintain a calm professional demeanor and make independent judgments. * Ability to maintain composure and work objectively with participants who may be hostile, verbally abusive, or under the influence of drugs or alcohol. * Ability to be a detail oriented, flexible, dependable, positive team player. * Proficient in Microsoft Office software (Excel, Word, PowerPoint, Access and Microsoft Outlook) and electronic medical records, including Centricity and preferably EPIC.
