AuthoraCare Collective is currently seeking a Hospice RN Long-term Care Case Manager for our Greensboro campus supporting High Point, Kernersville and Winston-Salem service areas. This position is Full-Time, Monday - Friday 8:00am - 5:00pm. Job Summary: Manage the over-all plan of care for a case load of hospice patients in collaboration with other members of the interdisciplinary group (IDG). Assess and identify patient/caregiver problems, establish nursing goals, and interventions. Work with physician to establish medical plan of care for the patient. Establish and maintain therapeutic relationship with patient and caregiver, including providing emotional support and teaching. Work closely with other members of the interdisciplinary group and communicates effectively both verbally and in writing with team members, patients, caregivers, physicians, pharmacists, and other health care professionals. Document the outcome of care. Provides dietary counseling for patient care needs as indicated. Assume on call responsibilities as scheduled. Education/Experience: * Graduate of an accredited School of Nursing * Must be currently licensed as an RN in North Carolina and in good standings with NCBON. * Experience in an acute, hospice, home health setting, or long-term care setting a plus. Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
