Performs advanced technical nursing duties and assists with other related nursing duties including phlebotomy. Involved in providing conducting patient admissions collecting patient medical history, including substance use disorders and mental health illness; assessment will also include history of infectious disease, chronic disease, involved in providing patient care in substance use withdrawal management, infectious disease, chronic disease, counsels on disease management, refer to medical homes.  Performs work under the administrative supervision of a licensed physician and/or the immediate supervision of a disease intervention manager. ESSENTIAL DUTIES & RESPONSIBILITIES The intent of this job description is to provide a representative summary of the major duties and responsibilities performed by employees in this job. Employees may be requested to perform job-related tasks other than those specifically presented in this description. * Conducts early detection screening services including phlebotomy * Stabilize clients according to medical protocols * Communication with staff of all significant information, which includes but is not limited to client mental status, vital signs, medications and my medication * Maintains the confidential nature of all patients and facility activities  * Documents client progress/regress according to established policies and procedures * Performs clinical nursing activities in a safe, professional manner * Obtains lab specimens as ordered by doctor  * Supervise shift nurses * Ensure that shifts are always covered  * Participates in interviewing of nursing staff  * Facilitate monthly staff training or as needed  * Part of the Audit Staff for Medical Reviews and will be trained to audit, prepare for, and sit through on-site audits * Attend and participate in interdisciplinary staff meetings * Ensuring that Intake Team completes all admission forms * Participates in Interdisciplinary Treatment Plan meetings * Ensuring that Intake Team completes all admission form * Participates in Interdisciplinary Treatment Plan meetings * Complete nursing assessments * Chart in the client medical record * Monitor vital signs according to policies and procedure, conduct patient checks and observe monitor or assign LVNs to assist with the above  * Review med pass compliance reports where applicable and report findings to supervisor * Reviews all lab results and communicate results with medical director * Reports to supervisor any administrative concerns * Oversee Doctors Orders for compliance and accuracy  * Oversee all medication administration for complaince and accuracy, inclusive of documentation * Complies with all polices and procedures in accordance with State Licensing Standards and Joint Commission regulations * Maintains required training and continuing education  * Serves on committees as appointed to by Director * Facilities team interaction ofr discharge planning * Collects and records data in a comprehensive, accurate and systemic manner * Evaluates the impact of therapeutic interventions upon the therapeutic milieu * Knowledge of program policies and procedures * Collaborates with the medical staff to provide medical and nursing care * Assists in formulating standards for patient policies and procedures * Promotes good public relations between program and nursing/direct care team * Maintains compliance with established nursing care standards * Supervises and instructs patients in medication self-administration (the preparation and giving of injections) as ordered by doctor  * Supports and assists with the orientation of new employees * Serves as a resource in speciality to other team members * Manages time effectively for proper balance between administrative responsibilities and other program requirements * Assists in formulating standards of care for patient care as required by State licensure requirements  * Reviews, revises, and formulates policies and procedures specific to program, with approval of the Medical Director and Performance Improvement Coordinator * Evaluates patient care daily by assessing patients and reviewing nursing documentation * Supervises all clinical nursing activities, assuring comfort and safety of the patients * Ensures continuity of care through the use of fore sight in directing, controlling, planning and organizing patient care * Expected to report, without the threat of retaliation, any concerns regarding policies and producers on patient privacy and any observes practices in violation of the policy to their supervisor. * Coordinates drugs testing for patients; follows mandated protocols; maintains accurate and complete records * Assists in blood draws, blood results consultations, in-body test, rapid COVID-19 test, rapid strep test, rapid influenza A/B test, rapid urine analysis, albuterol aerosol treatment, pregnancy test, intravenous hydration, glucose checks bloods pressure checks, electrical nerve stimulator treatment, ear wax removal, and eye washing * Evaluates and monitors patients until EMS arrives * Performs other related work as required.
