Nurse Family Partnership RN
We are seeking a part time (20hrs/wk) maternal/child community health RN to implement the evidence-based Nurse Home Visitation (NFP) model, Nurse-Family Partnership. NFP nurses are part of a movement of caring professionals using the whole range of their Nursing expertise. Nurses deliver health education services to expectant individuals and their families within a home-based setting for two and half years. The Nurse Home Visitor will offer prenatal education, nutritional guidance and support in setting and achieving health-related goals to achieve a healthy pregnancy outcome, optimal child health and development and economic self-sufficiency. Responsibilities include educating participants on infant care, early childhood development and promoting positive attachment and parenting. Our Nurse-Family Partnership program caters to a diverse population, particularly those affected by economic disparities. This part time position carries a caseload of 13 families at a time. This department has a very supportive team culture. Each Nurse Home Visitor meets with their supervisor weekly for reflective supervision. Purpose: Provide comprehensive community health nursing services to women and their families involved in the Nurse Family Partnership Program. This will be done in a manner consistent with efficiency, cost effectiveness and regulatory compliance. Responsibilities: * Accurately assesses own learning needs and develops strategies to meet them.Stays informed of current health care developments to provide safe, quality, nurse home visiting services.Establishes and records self-progress toward annual professional development goals. * Maintains clear, effective, open, honest communication with both internal and external customers and colleagues.Creates, maintains and supports a safe environment for open discussion.Seeks and responds appropriately to feedback. * Formulates nursing diagnosis based on nursing assessment and client goals.Evaluates client progress toward stated goals and NFP outcomes. Plans home visits in accordance with client goals and outcomes.Manage and prioritize own caseload for safe, effective care according to acuity/needs of clients.Prioritize clients for emergency preparedness.Maintain productivity standards established for the program.Utilize nursing expertise and professional judgement to inform appropriate healthcare provider of any pertinent change in condition of client. Actively engages in professional development to meet all NFP Nurse Home Visitor competency requirements.Meet with NFP Nurse Supervisor weekly for clinical supervision.Utilizes reflective practice in supervisory sessions. * Supports policies, procedures, NFP Visit-to-Visit Guidelines and standards of NFP and Home Nursing Agency.As requested/required, participates in community coalition in support of NFP.As requested/required, promotes public awareness of NFP.Develops and maintains community relationships to support client referrals. * Understands, supports, and coaches others in the NFP vision, mission and model.Represents the NFP vision, mission and model in actions and verbally to both internal and external customers and colleagues.Assists in creating a positive work environment that promotes productivity, mentoring, teamwork, and cooperation.Elicits and considers differing viewpoints when analyzing issues.Recognizes accomplishments of team members. * Position requires travel to various locations.Must be able to work a flexible schedule including some evenings and weekends. Must be able to travel by air to Denver, CO for education sessions.Provides home visits to women and their families eligible for NFP services.Adheres to nursing process and the NFP model of home visitation.Carries a caseload of up to 25 clients. * Schedule joint home visits with NFP Nurse Supervisor every four months.Attend and participate in bi-weekly case conferences and team meetings.Maintain quality, timely and appropriate comprehensive documentation both written and data entry into Efforts To Outcomes (ETO) software and Agency electronic health record.Provide information to support staff for timely and accurate data input to the ETO.Participate in review and analysis of ETO reports for achievements and areas for improvement.Participate in quality improvement efforts.Maintain confidentiality.Promote safety awareness for client/family members in their environment.Responsible for meeting/exceeding customer service initiatives.Initiate activities to report and/or resolve customer complaints.Adhere to infection control principles, standards, policies and procedures.Perform related duties as assigned or required. * Completes all required Agency/NFP Initial and Continued Education/Training Sessions.Maintains Agency-specific and NFP clinical competencies.Attends, participates and maintains certification in Dyadic Assessment of Naturalistic Caregiver Experience Training (DANCE).Develops therapeutic relationships, utilizes concepts of reflection and motivational interviewing with women and their families in a home visiting environment.Performs home visiting in accordance with the NFP model and NFP Visit-to-Visit Guidelines.Follows nursing process in implementing NFP into nursing practice.Ongoing assessment of physical, emotional, social and environmental strengths and challenges of women and their families as they relate to the NFP domains from prenatal period until the child's second birthday.Assists women and their families in establishing goals and outcomes which are individualized, client-specific, measurable and realistic.Provides education, support and referral resources in assisting women and their families in attaining their targeted goals in the NFP domains.Consults and collaborates with other professionals involved in providing services to women and families. Qualifications: BSN required. 2 years experience in maternal/child health, public health, home visiting, mental/behavioral nursing, or relevant experience. Licensure, Certifications, and Clearances: Registered, certified or licensed as required by state law for the profession. NFP RNs typically hold a BSN. ASN prepared RNs are only considered if they have received special dispensation by authority of the Nurse-Family Partnership? National Service Office on recommendation of the State Nurse Family Partnership Consultant. NFP approved ASN prepared RNs must agree to successfully completing an accredited BSN program and receive a BSN degree within three (3) years of hire.CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire * Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR) * Registered Nurse (RN) * Act 31 Child Abuse Reporting * Act 33 * Act 34 * Act 73 FBI Clearance * Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
