About
The Occupational Therapist Registered (OTR) certification validates the knowledge and skills of entry-level occupational therapists. The National Board for Certification in Occupational Therapy (NBCOT) offers the OTR exam, accredited by the American National Standards Institute (ANSI) and the National Commission for Certifying Agencies (NCCA).

To be eligible to take the OTR exam, applicants must meet the following requirements:
Have an entry-level occupational therapy degree from an ACOTE-accredited OT program
Agree to follow the NBCOT Practice Standards and Code of Conduct

Course Format
Exam Format
The OTR exam includes 170 scored questions. Candidates have 4 hours to complete the exam and need a minimum scaled score of 450 to pass. OTR exam topics include:
Evaluation and Assessment (25%)
Analysis and Interpretation (23%)
Intervention Management (37%)
Competency and Practice Management (15%)
Test Dates
Pearson VUE Test Centers offer OTR exams year-round based on availability at individual test centers.
Register for the Exam
Applicants register for the exam after receiving an Authorization to Test (ATT) notice from Pearson VUE. They can register online.

Click “Schedule” on the left column
Select country and state
Click “Next” under Identification Policy
Select the box next to “I confirm” and “I consent” and click “Next”
Enter eligibility ID and first 4 characters of last name
Search for a testing center
Select a test date
View the Pearson VUE Testing Center location page to find the nearest testing center.
Exam Day Instructions
Arrive at the testing center 30 minutes before exam time
Bring 2 forms of identification
The handbook contains a list of acceptable forms of ID
Applicant’s first and last name must exactly match the name Pearson VUE has on file
Examinees may not bring personal items, calculators, phones, notes, food or beverages into the test center
Examinees receive an email when NBCOT posts their scores online.

Applicants receive an Authorization to Test (ATT) after application approval. The ATT is valid for 90 days and includes information for scheduling the exam with Pearson VUE.
Renewal
OTR certification is initially valid for 4 years, but subsequent renewals are valid for 3 years. Renewal occurs in January through March, regardless of when OTRs earned their initial certification. The final application deadline is March 31. To be eligible for renewal, applicants must:
Obtain 36 competency assessment and/or professional development units
Complete the renewal application
Pay the renewal fee

Renewal Fees

Applications submitted after March 31st of renewal year: $25 late fee
Reinstatement of certification within one missed renewal cycle: $125
Reinstatement of certification with more than one missed renewal cycle: $175
Continuing Education
Applicants must accrue 36 units during the renewal period. The units are a combination of competency assessment units (CAUs) and professional development units (PDUs) in the following categories:

Frequently asked questions
What if I fail the OTR exam?
If you fail the OTR, you may retake it after designated waiting periods. Examinees must wait 30 days between exams for second and third attempts, wait 60 days between exams for fourth through sixth attempts, and wait 6 months between seventh and greater attempts. NBCOT requires a new application and exam fee with each attempt.
Can I still renew the OTR if my certification is expired?
Yes, you can reinstate your OTR certification with the submission of a reinstatement application and a fee of $125. If two or more renewal cycles have passed, you must pay a $175 fee.
Can I cancel or reschedule the OTR?
Yes, you can reschedule the OTR exam up to 30 days before the exam with no fee. If rescheduling within 29 days or less of the exam, Prometric applies a $35 fee. If rescheduling less than 5 business days before the exam, Prometric applies an $85 fee and it must issue you a new ATT.
Will NBCOT send my OTR score to a state regulatory board?
NBCOT will send your OTR score to a state regulatory board for a $35 fee per state. You can request a score transfer on your exam application.
Can I take a break during the OTR exam?
Yes, you’re permitted a break, but the time used to take breaks is taken away from the time allowed to complete the exam.

The National Board for Certification in Occupational Therapy (NBCOT) also offers the Certified Occupational Therapy Assistant (COTA).
