  ResponsibilitiesThe Clinical Care Coordinator serves as an assistant to the agency Executive Administrator, Executive Director, Director of Clinical Operations, and/or Patient Care Manager to ensure accuracy of patient electronic medical records information and documentation of hospice agency care processes. <ul><li> Assists with phone calls to and from field staff, physician offices, referral sources, patients, and residential and healthcare facilities. Takes messages and receives physician verbal orders, as needed.
