  Responsibilities 
 As the RN Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient, while continuously evaluating personal and professional performance and making necessary changes to increase productivity and quality of care delivered.  


  


 <ul>
 <li>Makes the initial nursing evaluation in determining eligibility for hospice services during visit within forty-eight (48) hours of referral. </li>
 </ul>


 <ul>
 <li>Identifies the patient's/family's physical, psychosocial, and environmental needs and re-assesses as needed, no less than every fifteen (15) days. </li>
 </ul>


 <ul>
 <li>Initiates and coordinates the plan of care while maintaining accurate and relevant clinical notes regarding the patient's condition. </li>
 </ul>


 <ul>
 <li>Documents problems, appropriate goals, interventions, and patient/family response to hospice care. </li>
 </ul>


 <ul>
 <li>Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care daily. </li>
 </ul>


  
 Education and Experience Experience Requirements

 <li>One year of clinical experience.</li>
</ul>
License Requirements
<ul>
 <li>Current RN licensure in the state of practice and one year of clinical experience.</li>
 <li>Current CPR Certification.</li>
 <li>Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.</li>

  Responsibilities 
 The Patient Care Manager -Hospice assists Executive Administrator/Executive Director/Director of Clinical Operations with all patient care activities to ensure the delivery of safe and efficient patient care which adheres to relevant standards of accepted nursing, regulatory, medical practice, and payer source criteria.

 <li>Responsible for compliance with hospice policies and procedures and with local, state, and federal regulatory requirements governing the practice of the assigned team. </li>

  Responsibilities 
 
 The Executive Director (Registered Nurse, RN) in Hospice supervises all aspects of patient care, all activities of professional staff and allied health personnel, regulatory requirements compliance, financial performance, and quality assurance performance improvement activities for agency. The Executive Director or alternate will be available on-site during business hours and additionally, if needed.

 <li>Directs the daily operations of the agency and acts as the Care Management Process driver. The Executive Director or designee RN, such as a Patient Care Manager, will receive daily reports on patients from staff. This process also includes review of patient care paperwork, referral information, recertification processes, discharge information, and event reporting, etc. daily. </li>

 <li>Reviews monthly financial results relative to all aspects of the operation to assure that quality patient care is delivered in the most cost-effective manner. </li>
 </ul>

 <li>Participates in the QAPI planning and processes, reporting, and improvement action plans as indicated. This includes chart audits, patient satisfaction, financials, contracts, patient/family complaints, etc. and appropriate follow-up.  </li>

 Education and Experience License Requirements
<ul>
 <li>Must be a licensed physician; licensed registered nurse; licensed social worker; or a Bachelor's degree college graduate with at least three (3) years of documented success in discipline/field of study and a minimum of one (1) year of full time experience in a hospice, home health, or other health care delivery system setting.</li>
 <li>Three (3) years of health care leadership preferred.</li>
 <li>Current CPR certification required for Executive Directors who may provide services to patients in the field.</li>
 <li>Current Driver's License and vehicle insurance, and access to a dependable vehicle, or public transportation.</li>
